STATE OF ILLINOIS
COUNTY OF WASHINGTON

      On this the 20th day of December A.D. 1897 personally appeared before me a
notary public, duly authorized to administer oaths within and for the county and state
aforesaid., ROBERT GUTHRIE, Aged 61 years, residing at Nashville, Illinois, who
being by me duly sworn according to law, declares:-
      That he is a pensioner of the United States as a private in Co. F. 10th, Regt. Mo.
Infty, by Certificate No. 448444, enrolled at the Chicago Pension Agency at the rate of
$16.00 per month, being granted for Falling Fits, or Epilepsy. That with the increase of
age the attacks are more frequent, has partial loss of sensation in lower extremities, and
unsteady in his gait, with a feeling of dullness and dizziness when he tries to work, and a
feeling of impending danger. Always in a condition of anxiety, so great is the mental
strain that I am compelled to refrain from any efforts at labor, being a brick layer,
necessary compelled to work on high walls or scaffoling. The Epileptic Attacks are three
or four times each month. That on account of the increase of the above described
disabilities he believes himself entitled to an increased pension. He therefore requests an
order to appear before some United States Medical Examining Board for examination,
that his present physical condition may be known. That he hereby appoints N.F. Jones of
Nashville, Illinois his true and lawful attorney to prosecute his claim. That his P.O.
address is Nashville, Illinois.
